Are You Ready To Start School Checklist

Must Haves

Immunization Records:

Immunization records must be turned in at the time of registration.

Please contact your child's healthcare provider to get this record and ask them to check that your child is up to date.

If not up to date, the school nurse will reach out to you to let you know which immunizations your student needs in order to start school.

Physicals Needed

All new students need to turn in a completed physical examination form completed and signed by the doctor within 30 days of registration.

Please contact your child's doctor to request this. Blank forms are available on the nurse's webpage.

Flu Shot Needed for Prek

All pre-k students under 5 years old, will be required to have proof of the flu shot given between 9/1/24 and 12/20/24 in order to return to school after winter break.

Please schedule early to avoid last minute issues. New between 1/2 and 3/31 must have flu shot proof.

To start school

1.Completed Physical Form (the exam must have been within the past calendar year   365 days of time being turned in) This is due within 30 days of registration. Form on website under HEALTH FORMS.

2. A complete, Up-to-Date Immunization Record is required upon registration.

3.If your child requires medication, has a medical condition, or has food allergies etc, please reach out to the nurse prior to school starting (over the summer via email). Email Christa Taylor (medical forms are available on the nurse's webpage under Health Forms).

** If your child has an upcoming physical that will cause the form to be more than 30 days to be turned in or for immunizations that are overdue, please contact the nurse.  If need resources for physical or immunizations please see the resource page here, or contact the nurse. Thank you! 😊

First Day and On: 

1. A change of clothes in a bag labeled with child's name written into them (sweatpants, tshirt, socks, underwear) needs to be sent to school. 

2. If your child has any updates on medical conditions please notify the nurse. x5311 or Email Christa Taylor.

3. If your child will be absent from school, please call 1-856-652-2700 and follow the prompts for J. Harvey Rodgers School reporting an absence. Please leave your child's name, teacher, and reason for absence. 

4. Please note that preschool students under age 60 months (5 years) are required by NJ to have proof of the flu vaccine between September 1 and December 31, 2024. The last school day of the 24-25 year before winter break is 12-22-24 . Must have this turned in by 12-22-24 in order to return to school in January.

5. Any Pre-k students starting Jan 1 through March 31 will need proof of flu shot before starting school.
