Must Haves
Immunization Records
All immunization records must be submitted at the time of registration. Please contact your child’s healthcare provider to request an updated immunization record and confirm that your child is current with all required vaccines.
If your child is not up to date, the school nurse will contact you with information about which immunizations are needed in order for your child to begin school.
Physical Examination Requirement
All new students are required to submit a completed physical examination form, signed by a licensed healthcare provider, within 30 days of registration.
Please contact your child’s doctor to schedule an appointment, if needed. Blank physical forms are available on the nurse’s webpage on the district website.
Flu Vaccine Requirement for Pre-K Students
Per New Jersey state regulations, all Pre-K students under the age of 5 must submit proof of receiving the influenza vaccine between September 1, 2025, and December 20, 2025, in order to return to school after winter break.
Families are strongly encouraged to schedule appointments early to avoid last-minute issues.
Any Pre-K student enrolling between January 2 and March 31, 2026, must also provide proof of a current flu vaccination upon registration.

To ensure a safe and healthy start to the school year, please review the following important health requirements:
Completed Physical Form
A physical exam dated within the past 365 days is required. This form must be submitted within 30 days of registration.

Medical Needs or Conditions
If your child has a medical condition, requires medication during school hours, or has allergies (including food allergies), please contact our school nurse prior to the first day of school, preferably via email over the summer. Email Nurse Patricia Scappa at pscappa@gpsd.us.

If your child has an upcoming physical exam that may delay submission or is behind on immunizations, please reach out to the nurse in advance.
