Immunization Requirements
Under NJ State law, NJAC 8:57-4, all students attending public or private schools are mandated to be immunized. If there is a communicable disease outbreak and your child is not fully immunized, you may be asked to keep your child home to protect the health and welfare of all students and staff within the school.
If your child turns 11 years old after school starts, he/she has two weeks to complete the mandated shots. Do not delay, have your child vaccinated today! See the list of available locations below where you may have your child immunized.
Sixth (11-year-old shots required to enter 6th grade):
Tdap - 1 dose - given after 11th birthday
Meningococcal - 1 dose - given after 11th birthday

Healthcare Provider (HCP) - If your HCP is out of the vaccine serum, request a script for the pharmacy to fill then return to your child’s HCP and have the HCP’s nurse administer the shot.

No insurance? Call Gloucester County Dept. Health (GCHD) at 856-218-4101 to make an apt. and they will administer vaccinations, FREE of charge. *Take a previous vaccination record with you to your appointment!
How do vaccinations protect my child’s health?
Immunizations help your body to fight off infections by training your immune system to recognize and attack germs.
They help the body defend itself from disease without dangers of full-blown infection.
*Everyone should get the recommended vaccinations at the recommended times.

How do I ensure the school is aware of my child’s vaccinations?
If you have a scheduled appointment, provide proof to the school nurse of the scheduled apt.
If you your child received the shots, provide a copy of the administered shots to your school nurse.

How should I go about obtaining a “Medical Exemption” and why?:
*Some individuals are allergic to certain vaccinations but this is rare, so speak to your child’s HCP about this.
A valid Medical Exemption must be written by your child’s HCP and a copy given to the school nurse.
Keep in mind, this record is not permanent.
This record must be reviewed yearly or even periodically by your child’s HCP.
How should I go about obtaining a “Religious Exemptions” and why?:
Please understand that although the shots may not be available at your child’s HCP facility, this does not exempt your child from meeting this code upheld by the NJ Dept. of Health!
This mandate must be met in some form or fashion!
If you are in a time crunch, and have exhausted ALL locations and options to obtain the mandated vaccinations, this may be a temporary fix to meet the immediate mandate.
To obtain a Religious Exemption, the NJ Dept. of Health prefer the parent/guardian write a hand-written letter to opt out of the mandated vaccinations, only if this is your “Religious” preference.
The letter should consist of the following:
Student’s name.
Date you wrote it.
The word “Religious” in the body.
Parent/guardians Printed name, clearly.
Parent/guardians Signature.
Another option is to ask your school nurse for a Religious Exemption form to fill out.
Once you have the Religious Exemption in place, when your child obtains an immunization at a later date, the letter becomes invalid.
The Religious Exemption letter may be reinstated in the future, if needed.
The Religious Exemption should only be temporary if utilizing it to meet the mandate on time!
Please know that not obtaining the vaccinations will not protect your child from future illness, so please stay on top of, and follow through in obtaining your children’s shots!
