• Please remember to turn in a current physical (on the NJDOE approved form) to the nurse. 

• ALL athletes MUST have a cleared physical on file in order to participate for a team.

• Once your physical is cleared by the school doctor AND you are signed up through the Google Form, the athletic trainer will notify athlete and coach when he/she is cleared to participate.
