The Notice of Intent to Participate form must be submitted by parents of the (out of district) School Choice applicant to the school district in which they reside (also known as the home or resident district) by December 3. In accordance with state guidelines, it notifies the home/resident district of the student's intent to participate in a School Choice Program outside of that district.
