For 2023-24 Winter Sports: 

The winter season is fast approaching. Physicals are due to your child's school nurse by  11-1-23. Physicals received after this date will still be accepted but may encounter delays in processing/participating.  Physicals received late will cause student athlete to be held out of participating in practices until physical is approved by School Physician.   

Sports Physicals must be conducted within 365 days of the first scheduled day of practice.  If the examination is completed more than 90 days prior to the first practice session, the student and parent are required to complete a Health History Update questionnaire. 

All forms should be turned in directly to your child's school nurse.
