Physical Exams for New Enrollees (do not use for sports)
For all new enrollees, including those enrolling into school for the first time or from out of state or out of the country, the pupil’s parents/guardians have 30 days to provide evidence of a medical examination that includes the following:
1. Evidence of immunizations pursuant to N.J.A.C. 8:57-4.1 through 4.24;
2. A medical history, including allergies, past serious illnesses, injuries, operations, medications, and current health problems;
3. Health screenings including height, weight, hearing, blood pressure, and vision; and,
4. A physical examination.
Parents should be aware of the importance of obtaining subsequent medical examinations of the student at least once during each developmental stage: at early childhood (preschool through grade three), pre-adolescence (grade four through six), and adolescence (grade seven through 12). These examinations should be conducted at the child’s medical home in order to ensure continuity of care.

Please use the universal physical exam form when your child receives their yearly physical. (do not use for sports)
