• As of now physicals will be conducted as usual. It is important that you get to/ make an appointment with your doctor ASAP!!!!! These appointments will be filling up quickly!

    The due date for sport physicals will be 7-22-24

    You may also scan and email your physcial and health history to the school nurse at eperewiznyk@gpsd.us

     

    It is very important that each physical is completed in its entirety.  This includes the first page on the physical forms.  If parents check “yes” to any questions, such as “ Do you get more tired or short of breath more quickly than your friends” they must respond with a written explanation at the bottom of the form.  The physician that is completing the physical must include all required vital signs, visual screenings and signatures.  Students and parents should include all clearances and required asthma action plans and or orders for epipens and diabetic management during sports.
