• Transportation Waiver Form

    All parent/guardian requests to waiver transportation for the 2021-2022 school year must complete the Transportation Waiver Form.  A form is needed for each child that will be waived.  All completed forms should be emailed to the attention of Diane Veight (dveight@gpsd.us).

  • Alternate Transportation Form (Updated 2021)

    All requests for students to be transported to and/or from school to one (1) location other than home MUST be submitted in writing on a YEARLY basis.  Students must go to/from that location every day with no exceptions.

    PLEASE NOTE: This form MUST be submitted to the Transportation Department by AUGUST 1st prior to the start of the new school year in September.  NO REQUESTS submitted AFTER August 1st will be considered until TWO (2) weeks after the first day of school.

  • Formulario de transporte alternativo (en español)

    Todas las solicitudes para que los estudiantes sean transportados hacia y / o de la escuela a una (1) ubicación distinta de la casa DEBEN enviarse por escrito ANUALMENTE. Los estudiantes deben ir a / desde esa ubicación todos los días sin excepciones.

    TENGA EN CUENTA: este formulario DEBE enviarse al Departamento de Transporte antes del 1 de agosto antes del comienzo del nuevo año escolar en septiembre. NO SE SOLICITARÁN SOLICITUDES DESPUÉS DEL 1 de agosto hasta dos (2) semanas después del primer día de clases.
